An easy way to get rid of this is to try dropping an item on top of the possible duplicate. It's a real item if it swaps and a fake if it vanishes. You'll have to do with with all the duplicates to get rid of the fake.

The charms are fine since their shorter duration just means you can't solo things you used to anymore. Now you need a caster buddy to help you, which is how it should be.

I think it has to do with the caster lvl of your character. Basically, if you have one the buffs work correctly and if you don't they last forever. At least that has been my observation.
