I have a +2 constitution ring, another +1 constitution ring and a +1 constitution boots on my character. If I log out with this character and then log back in, my character is essentially dead (i.e. has negative hitpoints) I notice that there are messages in yellow saying that items can not be equipped due to status effect.
If I don't wear the rings and boots when I log out, there is no problem.
Similar types of things happen with +2 intelligence rings. I always lose a spell slot on log out and have to re-memorize when I log in.
What is going on? Is this a server bug or a game bug?

The behavior is intentional, but if you're losing more than just your CON bonus HP, something's not right and I'll take another look.

Re: Constitution Boots and Rings
I think the issue is the fact you are wearing 3 Constitution boosting items.
Stat boost do not stack in NWN2, the game only takes into accound the highest bonus to the stat.
I think the issue likely occurs due to script on login un-equipping and re-equipping all the CON boosting items. Likely you are taking hits to your HP from all 3 items, when you only had the boost for 1 item.
My suggestion, remove the +1 CON boosting items and that might fix the issue.

Re: Constitution Boots and Rings
There are a couple of things going on here, however the advice to not equip more than one ability boosting item at the same time is sound. As mentioned, ability bonuses do not stack.
There is also an OEI bug in here in which if you're wearing more than one CON item, you can continually lose hitpoints each time you unequip one of the items. I can't really fix that, but not wearing multiple items should make this go away.

On the other hand, losing your bonus spells when you swap or remove INT/CHA/WIS-boosting gear is the expected behavior. You are required to rest while wearing these ability-boosting items in order to receive bonus slots/spells from the higher ability modifiers they confer. If you remove the item, you'll lose the abilities afforded by the higher modifier until you re-equip them and rest. Mostly a consideration for multi-class builds that rely on gear in order to be able to gain access to their highest level spells (insert multiclassing pro/con discussion here).
You'll also notice this when you polymorph, since you lose the benefits of any magical items your were wearing in your human form when you change into a creature/monster. Sometimes people have to transition before they notice this on their character sheets.
